I would like to sign up for the patient portal.
  1. Go to the portal login screen and click "Create An Account."
  2. Enter your name, date of birth, zip code and email. Click "Continue."
  3. Complete all required questions and create a unique user ID and password for your account. The password must be at least 6 characters, must contain at least one number OR an upper case letter, and should not contain any special characters (#%&*). Click "Create Account."
  4. Congratulations! You have an account.

My practice sent me an invitation to join the patient portal. Now what?
  1. You will receive an invitation in your email account. Open and click "Sign up for an account."
    PLEASE NOTE: Parents or guardians must create individual accounts for each child. Every patient must have their own account. Due to new regulations, accounts cannot be shared.
  2. Enter your date of birth and zip code to activate your account. Click "Continue."
  3. Create a unique user ID and password for your account. The password must be at least 6 characters, must contain at least one number OR an upper case letter, and should not contain any special characters (#%&*). Click "Create Account.
  4. Congratulations! You have an account. Now just update your profile and click "update profile."

I am trying to create an account, but am prompted to enter an activation code. Why?

You probably received an invitation to join the patient portal from your practice previously, but did not take action. That's ok! Click on the link on the right side of your screen to have your activation code resent to you so that you can proceed with establishing your account.

I got a message that says I already have a "HealthKey" portal account. Why?

Although you haven’t logged into this portal before, you may have created an account with another patient portal powered by Medfusion. HealthKey is a tool that allows you to join multiple patient portals, powered by Medfusion, with one user ID and password. If you remember your user ID and password, you may simply login.

PLEASE NOTE: Parents or guardians must create individual accounts for each child. Every patient must have their own account. Due to new regulations, accounts cannot be shared.

If your first and last name, gender, date of birth, zip code and email address exactly match an existing portal account, you will receive an email contgaining your user ID and a list of the medical practices with which you have an active patient portal account. If you remember your password, you can simply login, otherwise click the "Forgot Password" link to reset your password.

If your information closely matches an existing portal account, you can verify your account by clicking on "Verify by Phone Number."

I forgot my user ID.
  1. Use the "forgot your user ID?" link on the portal login page.
  2. Enter your email address.
  3. Confirm your date of birth.
  4. Answer your secret question.
  5. Your User ID will be sent to your email address.
I forgot my password.
  1. Use the "forgot your password?" link on the portal login page.
  2. Enter your user ID.
  3. Answer your security question and develop a new password.
  4. Open your email and click the "confirm my password" link.
  5. Confirm your password.

I tried that, but don't remember my secret question. Help!
